Medical examinations in the United States are typically required for adjustment of immigration status (such as green card) cases (Form I-485), and may also be required for requests for “V” non-immigrant status (Form I-539). Essen Medical Associates’ Civil Surgeon is authorized by the U.S. Citizenship and Immigration Services Department to conduct the necessary medical exams and complete the required forms. We also offer the required vaccinations and blood testing for applicants that need to meet these requirements.

The results of your medical examination are confidential and are used for immigration purposes only. When required to do so by law, the civil surgeon may share your results with public health authorities.

Our services include:

    Completion of Form 693

    Vaccinations to prevent the following diseases:
  • Mumps
  • Measles
  • Rubella
  • Polio
  • Tetanus and Diphtheria Toxoids
  • Pertussis
  • Haemophilus influenzae type B (flu)
  • Hepatitis B
  • Any other vaccine-preventable diseases recommended by the Advisory Committee for Immunization Practices

  • Hansen’s Disease
  • Sexually Transmitted Diseases (chancroid, gonorrhea, granuloma inguinale, lymphogranuloma venereum)
